2009 Membership Process

Fee $75/year

Print and sign waiver application.
Make sure you read this form.
 Please bring the signed waiver form to an open gym or mail the form to the address below.
Fill out the membership application online; you can do the following:
Please review the membership guidelines.
Payment is by check or PayPal. ***No Cash***
If paying by check, make check payable to TEAM DC BASKETBALL.
Checks may be mailed to: Team DC Basketball, P.O. Box 11103 Washington, DC 20008.
