2008 Membership Process

Fee $75/year

Process:

a. Print and sign waiver application. Make sure you read this form. Please bring the signed waiver form to an open gym or mail the form to the address below.

b. Fill out the membership application online; you can do the following:

• Email: If the submit button does not work then save a copy and follow the instructions on the application. You can either email the membership fee or bring it to an open gym.
• Mail: Send a completed copy of your application to the mailing address located at the bottom of the form.

c. Please review the membership guidelines.

d. Payment is by check or PayPal. ***No Cash*** If paying by check, make check payable to TEAM DC BASKETBALL. Checks may be mailed to: Team DC Basketball, P.O. Box 7490, Washington, DC 20004.

Can be used to pay registration fees for tournaments as well.
